I am a preferred provider with KAISER and San Andreas Regional Center of Santa Clara. I will submit claims to your insurance company on your behalf and automatically collect copay, deductible, and co-insurance at the beginning of each month.

I am out of network provider for all other insurance companies: Payment is automatically collected in full at the beginning of each month. A monthly itemized invoice will be provided for you to submit to insurance for reimbursement.
